Legal Consultation - AllOne Health provides one free 30-minute office or telephone consultation per
issue. If an individual wants to continue to work with that attorney, he or she receives a 25%
discount on his or her hourly rate.

Mediation - Each member is entitled to one 30-minute office or telephone consultation per separate
legal matter at no cost with a mediator. In the event that the member wishes to retain a
participating mediator after the initial consultation, they will be provided with a preferred rate
reduction of 25% from the mediator’s normal hourly rate. Typical mediation issues include divorce
and child custody, contractual and consumer disputes, real estate and landlord tenant issues, car
accidents and insurance disputes.

“Do It Yourself” Legal Forms Document Preparation - Our simple and inexpensive online process
enables consumers to complete their own legal document preparation from the comfort of their home,
without incurring the cost of an attorney, or dealing with lengthy completion and delivery periods.
Simply answer a few questions online and your documents will be prepared within 48 hours. Members
receive a preferred discount of 10% off and the types of forms available include divorce, wills,
living wills, power of attorneys, immigration and much more.
Financial
The financial benefit provides employees and household members with financial referrals to top rated
financial professionals. Each employee and household member is entitled to a free telephone
consultation with a financial counselor. Typical matters to call about include credit counseling, debt
and budgeting assistance, tax planning, retirement and college planning questions. Experienced
financial professionals and licensed CPAs provide these services.
In the event a member wishes to retain their financial professional for additional services, members may
elect to continue working with their professional at a discounted rate. This service provides the member
with unlimited access to their financial coach and the member may continue on a month to month basis.
Members are also entitled to receive a 30-minute income tax planning related consultation per year on
each separate tax issue they encounter. A CPA prepares all personal income tax documents at a preferred
rate reduction of 25% from the CPA’s normal fee.
Fraud Resolution and Identity Theft Program
AllOne Health’s Fraud Resolution and Identify Theft Services assist consumers at the inception of a
fraud-related emergency. Our affordable and expedient process:
- Provides members with unlimited access to a highly trained Fraud Resolution Specialist™ that conducts 10 emergency response activities within 48 hours.
- Assists members with restoring their identity and good credit.
- Provides members with a free ID Theft Emergency Response Kit
- Administers the costly steps to dispute fraudulent debts as a result of ID theft.
- Offers ID theft document preparation to assist the member in managing an ID Theft loss, such as forms and letters to creditors, credit reporting agencies and regulatory bodies.
- Provides legal or financial counseling related to identity theft issues.
- Follows-up with the member and monitors the fraud resolution progress.
Our fraud resolution program focuses not only on the immediate fraud related issues that a member is
facing, but also educates the member on preventative steps to ensure that they will not be victimized
again. By utilizing our Fraud Resolution Program, members will receive assistance from a trained
specialist that can assist the member in expediting their fraud claims and restoring their good credit.
Our professional staff and highly trained specialists are available to listen to your issues, answer
your questions, and assist members with restoring their identity and good credit. Studies show that if
